Complete and sign the registration formWhat you'll need
- Your child's name and date of birth
- Their team / age group
- Your address, phone and email
- Two emergency contacts
- Any medical or allergy details
- About 5 minutes
Players aged 12 and over should also sign to show they agree to the Player's Code of Conduct.
1. Player's Code of Conduct
- Play by the rules – the rules of your academy and the laws of the game.
- Never argue with a referee or other official – without these people you can't play football.
- Control your temper – verbal abuse of officials or other players doesn't help you enjoy or win any games.
- Be a team player – it's a team game, treat it that way.
- Treat all players as you would like to be treated – fairly.
- Co-operate with your coach, the referees and your team-mates.
- Play for your own enjoyment and to improve your skills and fitness.
- Never make remarks about anyone's race, religion, gender, sexuality or disability – you'll let down your coach, team-mates and family, and such comments can be illegal.
2. Parents & Supporters Code of Conduct
- Remember that you are there for the participants to enjoy the game.
- Always hand your child over to their coaches – make sure your child is in the care of the coaches and is not dropped off outside the area without adult supervision.
- Encourage participation but don't force it.
- Teach that enjoyment and participation are more important than winning.
- Never ridicule mistakes or losses – supporters are there to support, not to put players down.
- Lead by example and respect all players, coaches, referees and spectators – physical or verbal abuse will not be tolerated.
- Recognise all volunteers who are giving up their valuable time for free.
- Never publicly criticise referees or coaches – raise any concerns privately with academy officials.
- Never make remarks about anyone's race, religion, gender, sexuality or disability – you'll let down your family and yourself, and such comments can be illegal.
3. Agreement and consent
By registering our/my child with Gartcairn Football Academy, we agree to abide by these principles. We support the Academy in its undertakings and ask the Academy to take any necessary disciplinary action, where warranted, against any players, parents or spectators for repeated or serious breaches of these Codes of Conduct.
By completing the form you agree to your child taking part in the normal activities of the academy, and confirm that you have read the Code of Conduct for both players and parents/guardians and agree to abide by these values while representing the academy. Any serious or continued breach of these codes may result in your child being removed from the academy.
